A 56-year-old female. History of hypertension. CT abdomen: Detected right renal mass. 2.7 x 2.5 cm well defined medullary lesion involving inter and lower polar region of right kidney. Right partial nephrectomy has been performed. On gross: A fairly circumscribed, solid tan brown tumor with a central scar is identified measuring 1.8 x 1.7 x 1.6 cm. Areas of hemorrhage and necrosis are not seen. The tumor does not breach the renal capsule and does not infiltrate into the perinephric fat.

A 46 years male presented with history of renal mass with suspected malignancy.
PET-CT : The study reveals an ill-defined FDG and (SUV max : 21.96) exophytic heterogeneously enhancing soft tissue mass lesion arising from the posteromedial of the lower pole of right kidney measuring approximately 2.9 x 3.6 cms - suggestive of primary malignant pathology likely transitional cell carcinoma of kidney.
No scan evidence of distant metastasis.
Gross : Robot assisted right partial nephrectomy specimen measures 3.2 x 2.6 x 2.2 cms. Outer surface capsule is intact.
Cut section show a greyish brown to white tumor measuring 2.8 x 2.5 x 2.0 cms.
Primary HPE differential diagnosis :- 1) Eosinophilic variant of Renal cell carcinoma. 2) Oncocytic lesion.
Margins were clear.
